The Disability Identification section of a form provides a standardized way to collect OFCCP disability information, which is presented in the same form layout that the Department of Labor provides for candidates to complete. This information is beneficial in support candidates throughout the hiring process and on the job.
Use cases
- Clients who want to collect a candidate’s disability information to support them during the hiring process and on the job.
Disability Identification section
You will not be able to edit the form, the title displayed to the candidate, or the requirement. This section includes the following:
- Name (required)
- Date (required)
- Employee ID
- Disability information (required)

Once a candidate completes this section in a different language and submits the form, it will be translated to English before being added to the CEM and your integrated ATS, if applicable.
Add the Disability Identification section
You can add the Disability Identification section to a form by following the steps below.
- Navigate to the Forms page.
- Open the preferred form.
- Click Add Section.
- Select the Disability Identification section.
- The form will display the title and limited settings. You will not be able to edit the form, the title, or the requirement option, but you can click Preview Form to view the form and adjust some settings, if needed.
- Click Save.
- Publish the form.
View the completed section

When completed, this section of the form will be displayed independently in the Hire Details section of the Candidate Profile. Users that have full access to a candidate’s Hire Details section will be able to download the complete form in this drawer as a PDF.
