Candidate and employee forms include a combination of form sections and tasks. Learn about form tasks and their functions below.
Add new form task
Within each section, a user can add a task by scrolling to the bottom of the existing section and selecting + Add Task. You will then be presented with a menu that includes the a range of tasks to choose from.
Task options include:

Address – This allows candidates or employees to type their address and select the address populated by Google places API. The Address label will be editable and can be updated to better reflect the address requested, such as “Current Address.” This feature allows Paradox to capture the validated address, based on the country selected, for the candidate/employee to confirm accuracy.
- Note: Candidates/employees can still add an Apt, Floor, Suite value. Once completed, the address will be parsed and saved correctly by country selection. In addition to this, the first address task that is added to a form will be mapped by default to the Standard Candidate/Employee Attributes and will display inside the Hire Details section of the Candidate/Employee Profile along with any multi-response headings.
- Custom – This provides a blank question that can be configured to your preference.
- Date selection – This will allow you to set a topic (reason for date) and one or more date responses. In addition to this, you can mark this task as Sensitive Information and/or Hide from Manager to ensure more sensitive dates, such as birth date, are safeguarded from team members outside of your HR or Legal Team.
- Date range – This allows the candidate/employee to select a Start Date and an End Date as their answer. Candidates/employees can either type the date in the date range fields or they can click the calendar icon and select the date from the calendar that opens. If the End Date is the current date, candidates/employees can enable the Set end date to present toggle to reflect this.
- Display text – This can be used to provide further context, make announcements, relay additional information, etc. in the form. This does not create any new input fields for a candidate/employee.
- Document – This allows a user to import a file for the candidate/employee to view. A checkmark will appear after a candidate/employee has viewed the attached document.

Document upload – This will allow users to provide instructions to upload a document to the form.
- Note: The upload does not support .txt, .htm, or .HTML files.

Media – This task enables users add content such as video, images, or GIFs from the Media Library within the CMS. You can also map standard or custom attributes. Attribute-based media supports the same file types that can be uploaded in the Document upload task.
- Note: This is also available in the Document Builder when creating a Guided Document.

Fillable PDF – This allows you to upload a PDF that candidates/employees need to complete.
- Note: This is not available for Employee Forms.

Resume upload – This task will enable users to provide candidates/employees with instructions to upload their resume to the form. When uploaded, their resume will be converted to a PDF.

Notes:
- This is not available for Employee Forms.
- A resume upload can only be added to a section once and there is no options for multiple responses. The upload does not support .cvs, .pptx, or .xlsx files.
- Password Protected Resumes: Candidates/employees will receive an error when they attempt to upload a password protected resume. This error will be updated in the near future to indicate this issue is due to their resume being password protected, but it currently notes simply that an error has occurred.

Signature – This task collects a signature through either a Candidate Form or a Employee Form. Signature methods include the following:
- Draw
- Type
- Both
Candidate and Manager Signatures. If you want both the candidate and hiring manager to sign a form, you must configure the fillable PDF as both a candidate and a user form. If mapped correctly, all of the fields will be pulled onto the user form and the hiring manager will sign the same form.
Configurable task functions
Inside each section are tasks. Many of these have the same configurable functions associated with them. However, depending on what task is added to a form section, some of the configurations may not be available.
- Section name – This is the header that is displayed for each section. For every configured section in a form, there is a bubble that appears at the top and provides the candidate/employee with a rough estimate of their progress.
- Task name – This is displayed to the candidate/employee on their form to direct them on the information they should be providing.
- Maximum Responses Allowed – This allows the candidate/employee to enter in multiple responses for this task. A maximum of 20 responses are allowed. Click here to learn more.

Field type – The selected field type determines how a candidate/employee can input information in the form. This displays as a selector next to the open text box for Custom Question tasks. Options include:
- Short Answer
- Note: There is no character limit for this option.
- Paragraph
- Single Select
- Multi Select
- Drop Down
- Employee Lookup (only for employee forms)
- Location Lookup (only for employee forms)
- Note: When selected, the Assign Candidate’s Location and Available Job Locations Only settings will display.
- Short Answer
- Sensitive Information – Marking a field as sensitive will mask the information with ••• inside of the Hire Details section, the candidate’s/employee's conversation, and the Candidate Summary’s screening questions listed in the Candidate/Employee Profile. If a user has permission to view the Hire Details section and views a field marked as sensitive, they will not be able to view the field’s value unless they have the unmask permission. Hide From Manager – When a user has toggled this on for a field, it will be completely hidden from Hire Details.
- Note: The only way to access this information is through reporting or an integration.
- Add Confirmation Field – This will require candidates/employees to re-input their information a second time to limit the chance of error. This can be added to Short Answer custom questions and Date Selection tasks.
- Required – Toggling this on will make the field a requirement to fill out before proceeding to another section or completing the form.
